Southeast Asian ovalocytosis

A 52-year-old Malaysian man, a 24-year-old sub-Saharan woman, and a 28-year-old Madagascan woman (who was heterozygous for hemoglobin S) were admitted to North Hospital in Marseilles, France. Blood tests using an Advia2120i hematology analyzer (Siemens) showed no or mild anemia (109-150 g/L), normal or high mean corpuscular hemoglobin concentration (339-364 g/L), and borderline or slightly high red cell distribution width (15%-19.2%).

Study indicates a genetic test may show which chemotherapy patients are at risk for serious blood clots

Chemotherapy is known to carry a risk of venous thromboembolism in cancer patients, a particularly common risk in frequent cancers like breast cancer. But a genetic test may predict which of these patients are most likely to develop such serious blood clots, researchers report.

VTE is preventable through prophylaxis treatment with heparin, an anticoagulant. But because a side effect is less controllable bleeding, the drug is not routinely recommended to patients undergoing chemotherapy.

Researchers examined 4,261 Swedish women diagnosed with primary invasive breast cancer between 2001 and 2008. Risk stratification was based on chemotherapy use and genetic susceptibility, which was determined by a risk score assessing nine genes involved in VTE. Patients ranked in the highest 5 percent were classified as having a high genetic susceptibility.

Patients were followed for a median of 7.6 years, and 276 experienced a VTE during that time.

New method to restore function of white blood cells in septic patients

New research findings published in the Journal of Leukocyte Biology, suggest that treating the white blood cells of sepsis patients with antibodies that block programmed cell death-1 (PD-1) and programmed cell death ligand (PD-L1) molecules may restore their function and ultimately their ability to eradicate deadly bacteria.

"We hope that this study will lead to a better understanding of why patients with sepsis are often unable to successfully eradicate invading microorganisms," said author Andriani C. Patera, Ph.D. "Furthermore, we hope that this study will stimulate new therapies to treat sepsis based on stimulating various components of the immune system."

Platelet Satellitism

Platelet satellitism was first reported in the early 1960's. It is a rare condition that occurs when an IgG antibody forms in the presence of EDTA, the anticoagulant that is used for the collection of hematology blood specimens. The IgG antibody is directed against the glycoprotein IIb/IIIa complex on the platelet membrane. As the antibody coats the platelets, the platelets rosette around segmented neutrophils, bands, and sometimes around monocytes. Antibody-coated platelets that are huddled around white blood cells (WBCs) will not be counted as platelets by automated equipment and the platelet count will be falsely decreased. If a peripheral blood smear is reviewed, platelets will be observed attached to WBCs. The image on the right illustrates platelet satellitism with platelets adhering to a neutrophil.

Here we have a neutrophilic leukocytosis with a left shift (there’s a myelocyte, top left, a metamyelocyte, bottom left, and a promyelocyte, center) and a basophilia (right). Check out how mature the chromatin is in the myelocyte, metamyelocyte, and segmented neutrophil – but how fine it is (you can even see nucleoli!) in the promyelocyte). Also, the granules in the promyelocyte are in the cytoplasm and over the nucleus as well. The basophil granules are kinda obscuring the nucleus, but that’s what happens in real life, so we’ll call it artistic rendering.

Iron-Deficiency Anemia in Ulcerative Colitis

Many patients with ulcerative colitis don't receive recommended testing and treatment for the common problem of iron deficiency anemia, reports a study in the October issue of Inflammatory Bowel Diseases, official journal of the Crohn's & Colitis Foundation of America (CCFA). The journal is published by Wolters Kluwer.

The study used nationwide data on 836 patients newly diagnosed with ulcerative colitis in the Veterans Affairs (VA) health care system from 2001 to 2011. Over a median eight years' follow-up, 70 percent of patients developed anemia: low levels of hemoglobin, which carries oxygen in the blood.

The study focused on how many of these patients were tested and treated for iron deficiency anemia—a common complication of ulcerative colitis, caused by intestinal bleeding and malnutrition. Iron deficiency anemia has profound effects on health, including declines in physical and cognitive abilities.

The results showed "inadequate monitoring and treatment of anemia and iron deficiency" among patients with ulcerative colitis. Of the patients who developed anemia, 31 percent did not undergo recommended tests for iron deficiency. Sixty-three percent of patients tested were diagnosed with iron deficiency anemia.

Neutrophil disorders and their management

Neutrophil disorders are an uncommon yet important cause of morbidity and mortality in infants and children. This article is an overview of these conditions, with emphasis on clinical recognition, rational investigation, and treatment.

Neutrophil disorders are an uncommon, yet important, cause of morbidity and mortality in infants and children and should be considered when investigating children for immunodeficiency. They are especially likely when the clinical presentation includes features such as oral ulcers and gingivitis, delayed separation of the umbilical cord, uncommon infections such as hepatic or brain abscesses, uncommon organisms such as S marcescens or Pseudomonas spp, or when the individual has features of syndromic conditions associated with neutropenia or neutrophil dysfunction. All patients with recurrent oral infections, skin abscesses, perianal and perirectal abscesses, poor wound healing, sinopulmonary infections, or deep visceral abscesses should be evaluated for defects in phagocyte function. Appropriate investigations can lead to specific diagnoses, and general and specific management measures can reduce both mortality and morbidity and permit genetic counselling and antenatal diagnosis in some cases.

Blood Eosinophil Level Predicts Severity of Asthma

Asthma represents a significant clinical and economic burden to the US healthcare system. Along with other clinical manifestations of the disease, elevated sputum and blood eosinophil levels are observed in patients experiencing asthma exacerbations. The aim of this study was to evaluate the association between blood eosinophil levels and asthma severity defined using Expert Panel Report 3 guidelines.

Among 1,144 patients with an asthma diagnosis, 60 % were classified as having moderate-to-severe asthma. Twenty four percent of patients with moderate-to-severe asthma and 19 % of patients with mild asthma had an elevated peripheral eosinophil count (p = 0.053). Logistic regression showed that moderate-to-severe asthma was associated with 38 % increased odds of elevated eosinophil level (OR 1.38, 95 % CI: 1.02 to 1.86, p = 0.04).

Patients with moderate-severe asthma are significantly more likely to have an elevated peripheral eosinophil count than patients with mild asthma
